FULL-THICKNESS SKIN GRAFT USINE THE SKIN STRETCHED BY TISSUE EXPANDER



Abstract
Recently, there has been attempts to use the skin stretched by tissue expander in the field of skin graft. But there remained some questions to be solved such as ¢¥take¢¥ capability and the amount of postoperative contracture. In 1988, Namba and his coworkers described their clinical and experimental experiences in the use of full thickness skin graft that was transplanted after being stretched by tissue expander and concluded that there were no differences in the amount of postoperative contracture between the conventional full thickness skin graft and the full thickness skin graft using the skin stretched by tissue expander.
We describe here our clinical experiences and the advantages of the full-thickness skin graft using the skin stretched by tissue expander. We treated 13 patients of hypertrophic and contracted scars with the full thickness skin graft using the skin stretched by tissue expander. The expanded skins which were composed of full thickness skin and underlying loose connective tissue and preserved abundant upper subcutaneous vascular plexus were transplanted.
The results were as followings
1. The size of the harvested donor was larger than that of the conventional full thickness
skin graft donor : 1.5 to 2.5 times in width, 1 to 3 times in length.
2. ¢¥Take¢¥ process was nearly the same with the conventional full thickness skin graft. 3. Grafted skin showed no hardening, absence of pigmentation and normal texture.
4. Scarring of the graft margin was nearly the same with the conventional full thickness skin.
